Helping people navigate reentry, access essential services, and build a path toward long-term stability.
Meeting People Where They Are
Our approach is practical, respectful, and grounded in lived experience. We recognize that successful reentry looks different for every person. By helping individuals understand their options, create achievable next steps, and connect with the right resources, we work to make the transition back into the community more manageable and sustainable.
Funding Status: Rural Resilience Project is currently seeking funding to launch these services. The programs described on this page are proposed and will be offered based on funding availability.
System Navigation Services
Navigating services after incarceration can be complicated and overwhelming. We help individuals identify their needs, develop a personalized action plan, and connect with existing community resources.
Support may include:
-
Individual needs assessments and action planning
-
Case management and ongoing support
-
Connections to housing, employment, transportation, healthcare, and benefits
-
Referrals to behavioral health and recovery resources
-
Assistance navigating county and community-based services
-
Coordination with service providers to help reduce gaps in care and support
Our goal is not to duplicate services that already exist, but to help people successfully find, access, and stay connected to them.
Legal Services
Legal issues can create significant barriers to successful reentry. We help individuals identify legal needs and connect with appropriate legal resources and assistance.
Services may include support with:
-
Understanding and addressing outstanding legal obligations
-
Record-related and reentry legal needs
-
Identification and documentation issues
-
Referrals to qualified legal service providers
-
Navigating legal requirements that may affect housing, employment, benefits, or other opportunities
